Original Submission:  <b>After installing the header, I go to Setup/Config 
Admin Login. At the top of the page i see:</b>

<b>So now i when i click install all apps, I get:</b>

That was it. I mistakenly said "from host permissions" instead of "localhost"

Have you created the database and assigned the permissions to the phpgw user in 
mysql?
