The version mentioned is no longer supported, please upgrade to
the latest version.  If you still experience problems please
report it on our bug tracker at

I think that the setup/index.php is supposed to setup 
the header.inc.php, but it will not.  I'm running 
PostGreSQL and the latest version of PHP.  It says:
"You have not created your header.inc.php yet!
You can create it now."  

My Question: How do you create it?
