[PATCH v6 19/41] target/arm: Reorganize ARMMMUIdx

Subject: [PATCH v6 19/41] target/arm: Reorganize ARMMMUIdx

Prepare for, but do not yet implement, the EL2&0 regime.
This involves adding the new MMUIdx enumerators and adjusting
some of the MMUIdx related predicates to match.
