[qemu-web PATCH] update links to the SubmitAPatch wiki page

[qemu-web PATCH] update links to the SubmitAPatch wiki page |

The page is now part of the documentation, but it also has a redirect
in the qemu.org web server to provide a stable URL. Use it instead
of linking out to wiki.qemu.org.
