[PATCH v2 19/42] i386: Rewrite blendv helpers

Subject: [PATCH v2 19/42] i386: Rewrite blendv helpers

Rewrite the blendv helpers so that they can easily be extended to support
the AVX encodings, which make all 4 arguments explicit.

No functional changes to the existing helpers
