I did the following test:

(add-to-list 'backup-directory-alist
             (cons tramp-file-name-regexp "/tmp"))

Playing with "/sudo:address@hidden:/home/albinus/123" on my host gave
me a backup file "/tmp/!sudo:address@hidden:!home!albinus!123.~1~".
Would this be sufficient for you?
