new module 'signal-h'

But it is quite likely that we will want to wrap signal() alone. As a
preparation for this, let me rename the 'signal' module to 'signal-h'.
Like Eric did with the 'fcntl' module on 2009-08-21

        * doc/posix-headers/signal.texi: Update.
        * NEWS: Mention the change.
