"Smart" hiding of button in the GUD toolbar

I'm not convinced that the current "smart hiding" of various button in
the gud toolbar is optimal from a user's pov.

The rationale seems to be that if there is some other way to
do the task with the mouse, hide the button.

In particular, I think the following examples of "smart hiding" is
undesireable (i.e. I suggest to always show the icons):

Hide "toggle breakpoint" since we can do that by clicking mouse-1
in the fringe or margin.

Hide "run until" since dragging mouse-1 or clicking mouse-2 in fringe
does that.  

BTW, dragging only works for "what's on the screen" already,
so if the arrow is not on screen, or the target is off screen,
you cannot use the drag.  Besides, I find that it is somewhat
flaky.  So I suggest that we remove that particular feature,
which really seems quite superfluous.
