Concerning bulgarian support, there is no reason to include cp1251 in core too, because it's an alias for windows-1251, which is already in Emacs core.
There is something different between windows-1251 (as in core) and cp1251 (as in codepage.el) in choosing which exact X font to use. In fact, it's not that cp1251 successfully maps 1251 to the X font, but cp1251 successfully _replaces_ the selected "global text font" with another one that contains the needed characters. windows-1251 isn't so successful in doing so.
I have no idea about Emacs internals about handling fonts. Will you list some source files where I can begin to look for clues?
