I like a similar tagging system for certain parts of the
notes, say for assessment:

I type:
"palmar tendon induration, deficient extension Dig 2/3,
seems like Dupuytren left hand* to me"

* = At this point I press <SHIFT+CTRL> + three times <Cursor Left>
thus marking "Dupuytren left hand" followed by <ALT-D> to mark
this as a "diagnosis" which may or may not invoke predefined
actions for this diagnosis (billing, detailed documentation,
recall scheduling, operation scheduling, etc.). This way the
phrase is marked "this is an item to be used as a diagnosis
for coding for billing". Our current commercial system does
not do this but instead forces me to enter things more than
once at times because I already mentioned the diagnoses needed
for coding in the narrative.
