If security is your main concern and servers are available in general, would it 
help to protect the wiki pages by login/password which are only given to 
trusted parties ? That way it would be much harder for outsiders to even find 
out that there is a wiki at all. The gnumed collaborators could still 
contribute. The contents could be mirrored read-only (via, say, wget) so that 
everybody could still read the contents on static pages.
