Done.

Correct. The tooltips say as much, too.

No.

That is more like it: *if* you enter something into one of
those fields you need to enter something in the others as
well -- either a complete address or none.

No, but that happens, too.
Consider, for example, the "Address" field: Herein one can
search (by (part of) zip code or street name) for an
existing address and it will display a summary of the
address as witnessed in the first screenshot.
Upon selection of an existing address the other fields will
be filled in - see the second screenshot.
Yet, if you happen to edit (or select) another address field
the existing-address is cleared out and the patient will
have get a new address attached to herself. However, the
other fields stay pre-filled from the existing address for
ease of entry ;-) See 3rd screenshot (observe the redacted
zip code).

No.

Yes. It facilitates adding a primary phone number quickly.
