If you therefore add as a choice Group by: Substance

then the left-most column resulting should be Substance. It would here  
be possible to have

        Substance > Dosis > Handelsname > Plan

because in the case of a poly-pill, the dosis refers to the component  
while the Plan refers to the poly-pill

You could therefore make the argument that under the group order  
"Episode" it should be

        Episode > Substance > Dosis > Handelsname > Plan ...

and grouped by Handelsname it should be

        Handelsname > Plan > Substance > Dosis > ... Episode

I am taking pleasure in the suspicion that few EMRs may enjoy this  
open dialog, but at least some of the open source ones do :-)
