I take it you are talking
within and among procedures
within and among history, active problems etc?
There is an argument to present the items in some kind of time order. Doing so
as a general approach, across all types of items (where applicable) would
ignore that the EMR tree allows a custom order of display. Some specifics...

- I would suggest clin_when, because if I enter something today that describes
health 10 years ago the item is more clinically meaningful to be located
relative to the items that occurred immediately before and after
- the fact that I only just today created this item is important to *somewhere*

This would be most 'conventional'. One concern is that newer problems may be
out of view and require scrolling, but this is helped if

I would suggest
YYYY.MM <item name>
YYYY <item name> (where there is no month, such as PHx item entered by
age)
