one option could be a filtered list, displaying -- for each type of encounter 
recorded for the patient -- the

        clin_when
        encounter type
        provider (last signer, I guess)

for that most recent encounter of that kind.

Caveat is that it needs to be clear to the user that there could exist many 
more encounters of the kinds listed and which are not shown.

The alternative is to list all of the above, unfiltered, but maybe it is 
reasonable to think that anyone who wishes to see that additional detail would 
double-click to go to the Encounters view/edit area?
