Actually the labels do show a colon, it is the separation from the text that
causes the problem.

I misinterpreted "last" for its alternate meaning "final" or "single most
recent".
The alternate meaning would be to display the value e.g. the date of the most
recent single encounter, no matter the type.
Another alternate meaning follows the nearest preceding word (admissions) ... I
had in fact expected that it could be here that the most recent hospital
admission (where date of discharge is "null") could be displayed but I am
perhaps misremembering here. I do agree it is helpful to be shown, in the
contacts and inserted above the contact details, that it may be no use to try
to phone the patient at home or at work if they are currently admitted to
hospital.
Coming back now to the overall label name, what about
Chart and patient activity:
and to the right of it
(interval filter: [ ])
thus
Chart and patient activity: (interval filter: [ ])
because this would make more clear that it is a user input area... it is the
only such one in the entire plugin.
