I grabbed the fonts from CentOS 7.6 in

        /usr/share/fonts/default/Type1

and copied them onto my CentOS 7.9 system.

I then reran 'configure' in 1.22.4 with

        --with-urw-fonts-dir=/usr/share/fonts/default/Type1

as an argument, and reinstalled things.

I recreated the file

        bad-sqrt.pdf

using 'groff' as per usual, i.e. no special embedding. I do note that now I still have:

$ pdffonts bad*pdf

I note that it now works as previously, i.e. the long tail on the square root sign covering the entire expression has now reappeared.

Somebody needs to tell the CentOS team or whoever they are these days that they need to stop removing things from standard packages.

Out of curiousity, where on other people's systems are those '.pfb. files?
