It really depends on how and who is entering the data.

The initial person (party) model is for demographics. In large
institutions, where you have people dealing with demographics and Vital
Records Systems, you will have that team taking care of this part.

One important aspect on having entering demographics data separately
from the medical is data privacy. This way, the medical data from the
person is only accessible to health professionals that have
permission to it, and the health professional has already all the
demographics when the patient arrives to the encounter.

Of course, in small centers and private offices, the health
professional will do both demographics and medical info. In this case,
you can enter the information from the patient form.

In general cases, if you can, use the two-step way (one team for
demographics and one team for health).
