I finally end up doing something similar.

In .dir-locals.el:
`((nil . ((geiser-guile-binary . "./.geiser-guile"))))'

In .geiser-guile:
`
#!/bin/sh
exec guix shell --pure -D -f .guix/packages.scm -- guile $@
'
