(Maybe I'll also find something along the way about another long-standing wish of mine: being able to place LilyPond output to PDF layers (Optional Content Groups))

IIRC there has been discussion on this topic. It is technically related to table of contents, as they also introduce a PDF-feature, a clickable link, to the resulting file. The popup on hover would be another PDF-related extension. So you might have a look in the corresponding LaTeX packages and the toc-mechanism in lilypond. It should be possible, but I can't say what it would mean to actually build it.

would it seem feasible to insert tooltip-like items in the PDF file?
I am thinking about something similar to a BalloonText that doesn't get printed in the PDF but which appears in the viewer on hovering.
This would be great for annotating stuff in the files.
