Are you using a rpm install?  

Grab this file:
http://savannah.gnu.org/cgi-bin/viewcvs/*checkout*/phpgroupware/phpgroupware/header.inc.php.template?rev=HEAD&only_with_tag=Version-0_9_14-branch&content-type=text/plain

And save it as /path/to/phpgroupware/header.inc.php.template
