This issue seem to have been fixed by:

* faces.el (x-create-frame-with-faces): Only setup the toolbar if
tool-bar-mode is on.
but I had to back out that change later because it was causing problems
in compile.el. The compile.el problems are independent of this change,
and are described in bug#1133.
