I'll look into adding a continuous (unpaged) plain text output
format for the next release.  Irrespective of whether there are
bugs in the current plain text format or not (and if there are,
please tell me about them!), zero-width top and foot margins
don't give it to you because things can't straddle the page
boundary even though you can't see it any more.
