ChangeLog: 

* actionBring_all_to_front renamed to actionRaise_all_windows (labels, 
signals and slots changed accordingly)
* a new hidden separator instance for the Window menu to be able to insert
new windows in the exact place between the top "Raise all windows" action
and any further actions below the window list
* actually connect/disconnect the window actions so we can forward the
activation of each of them properly
