Not neccessarily.
One might decide to for example run the "adress book" service on his old 
e-emith box, separat e from all other services. For performance reaosns, they 
might decide to set up the "pharmaceutical service" locally one on each 
client. In those cases, the address service will be one instance to which 
many clients connect, while the pharma service will be one service instance 
per client instance.
