Did hardware diagnostics include CHKDSK? That includes a check for the file system's security descriptors.
And how about System File Checker? In Admin command prompt or Admin PowerShell,
sfc /scannowDoes temporarily disabling antivirus or security software make a difference? Doubtful, I think, but stranger things have happened.
