Don't know much about the PUBLISH method. Sorry.

a SUBSCRIBE with 0 close the subscription and you'll get a new NOTIFY.
(so it's also a fetch)

A simple fetch is a SUBSCRIBE with an expires set to a greater value.

I think it should contain the xml document.
