To get your app to use a custom title, put the following into 
app/setup/phpgw_en.lang.
app<tab>common<tab>en<tab>Application Name

Replace app with the unix name of the application, and Application Name 
with the name that you want to call the app (also, press tab instead of 
typing <tab>).
