Surface Button

Sent before, but no response, and in with a load of other stuff, and this
probably deserves highlighting.

Re: gui/surface_button and gui/gui_surface_button
this was changed because VC
doesn't allow files to have the same name in different directories in a
project. It seems surface_button got left in the CVS, and has been modified
a lot since gui_surface_button, but Windows projects still use the old file.
Can one copy be removed from CVS.

I know this was discussed a long time back, but it seems no definative
answer got reached.

It also seems that now there is only one surface_button.cxx, so VC++ would
actually now be happy to just use the normal surface_button.cxx, and have
gui_ removed.
