No.  You can write whatever output driver you want.  See the existing
output drivers, such as speechd-ssip.el or speechd-braille.el, for
examples.

However I'd strongly discourage making direct speech output drivers for
other than experimental purposes.  Enhancing or fixing Speech Dispatcher
is a much better way to go.
