I can not reproduce this, what window manager are you running?  Do you 
get the same behaviour if you disable and enable tool-bar-mode?

I also find it difficult to reattach the tool bar by positioning it in 
the right place.  If you have a window manager close button on the title 
bar of the detached tool bar, you can use that and then it reattaches 
itself.
