The verses should be laid out underneath the notes and the notes should be repeated for every verse/chorus.
If my song has a chorus and 2 verses it should look like that:

contains: the text and the notes of the first verse (the verse's syllables should be placed under each corresponding note)
-- thirds page --
contains: the text and the notes of the chorus (again the text is laid out under the notes)

contains: the text and the notes of the second verse (again the text is laid out under the notes)
-- fifth page --
contains: the text and the notes of the chorus (once again the text is laid out under the notes)
