Actually I don't think that would be very difficult.

A typical entry header looks like this:

@node acciaccatura
@section acciaccatura

ES: mordente de una nota,
I: acciaccatura,
F: acciaccatura, appoggiature brève,
D: Zusammenschlag,
NL: samenslag,
DK: ?,
S: ?,
FI: ?.

The script would need to extract the rest of each line beginning
@section, ES:, I: etc.  Remove duplicates, remove ?s, and
for each of the remainder insert a new line containing that extract
prefixed by @cindex.  Repeat for the next @node.
